Licensed Practical Nurse (LPN) – Hospice

Schedule: Monday–Friday, 9:00 AM–5:00 PM

On‑Call Rotation: Evenings (5 PM–9 AM) and weekends (shared rotation)

Setting: Home hospice visits (coordinates with RNs)

Location: Colorado Springs, CO

Position Summary

The Hospice LPN provides compassionate, patient‑centered care in the home setting, collaborating closely with RNs to ensure timely visits and high‑quality clinical support. This role requires strong documentation abilities, proficiency with EMRs, and excellent interpersonal skills.

Key Responsibilities

• Provide in‑home hospice care and complete assigned patient visits.
• Coordinate with RNs to ensure all patients are seen as scheduled.
• Rotate in the on‑call schedule (evenings/weekends) and make visits as directed by triage nurses.
• Document all care accurately in Netsmart EMR.
• Deliver compassionate, patient‑focused care and work collaboratively with the interdisciplinary team.
• Adhere to hospice protocols, safety guidelines, and infection‑control requirements.

Requirements

• Active LPN license (state-specific).
• Minimum 2 years of hospice experience required.
• Strong documentation skills: quick learner with EMRs (Netsmart experience preferred).
• Compassionate, team‑oriented, and dependable.
• Ability to participate in on‑call rotations.
• Flu vaccine required.

*Offered pay rate will be based on education, experience, and healthcare credentials.
